For referring practitioners

To ensure timely processing, all referrals must include:

• Date of referral
• Referring GP's name, address, provider number, and HealthLink ID
• Patient's full name, address, date of birth and contact number
• Reason for referral

If available, please also include:

• Patient's Medicare number, private health fund details, and email address

Referrals are reviewed and triaged by the doctors at Hollywood Gynaecology before an appointment can be booked. Once triaged, our staff will notify the patient when to contact the clinic to schedule an appointment.

Please note: this is a private practice and we are unable to offer appointments to patients without private health insurance.

1. Obtain a referralA current GP referral is required to access Medicare rebates for specialist consultations.
2. Triage & schedulingOnce your referral is received it will be triaged by the clinic, and our staff will contact you to arrange a suitable consultation time.
3. Private practiceHollywood Gynaecology is a private practice; appointments are only available to patients with private health insurance.
4. Before your visitBring a copy of your imaging, a list of current medications and any previous operation records.
